Angiomyofibroblastoma of the Male Para-Vesical Space Following Radical Nephrectomy for RCC: A Case Report

Abstract

Angiomyofibroblastoma (AMFB) is a rare benign mesenchymal tumor that predominantly arises in the vulvovaginal region of middle-aged females. It is essential to discern an AMFB as it is a potential mimicker of angiomyxoma which runs an aggressive clinical course. Here, we report an infrequent case of (AMFB) that arose from the left para vesical space in a middle-aged man who had undergone right radical nephrectomy for renal cell carcinoma two years back. The patient had left abdominal pain and showed a cyst measuring 17 × 20 cm on the radiological scan. After a multidisciplinary team discussion, the well-circumscribed tumor was completely resected and sent for histopathological evaluation. It comprised irregular anastomosing islands and cords of plump to spindled cells in a collagenous stroma with numerous thin-walled vessels and adipocytes. Immunohistochemically, the tumor cells were reactive for Desmin, CD34, CD10, CD99, S100 (very focal) and immunonegative for PAX8, SMA, MUC4, STAT6, Chromogranin, & MyoD1. Diffuse strong positivity of stromal cells for Estrogen and Progesterone receptors was noted. Surgical excision is the recommended treatment and is curative.
