Crisis Resource Management and Disaster Preparedness in Pediatric Trauma Care

This chapter provides a structured framework for primary care providers (PCPs) involved in pediatric trauma care during situations and mass casualty incidents (MCIs). Given children’s physiological, developmental, and psychological differences, responding to pediatric trauma requires tailored approaches that combine rapid assessment, efficient resource use, and coordinated teamwork. The chapter introduces the principles of Crisis Resource Management (CRM), highlighting the importance of environmental familiarity, anticipation, leadership, timely escalation, effective communication, situational awareness, and task delegation. A real-life scenario involving a child injured in an MCI illustrates how these CRM principles can be practically applied in high-stress environments. Special emphasis is placed on improving decision-making, reducing fixation errors, and optimizing team roles in real time. Communication tools such as SBAR and closed-loop communication are essential strategies for maintaining clarity and enhancing patient safety. In parallel, the chapter explores the elements of pediatric disaster preparedness, focusing on the need for pediatric-specific equipment, standardized triage systems like JumpSTART, caregiver reunification strategies, and emotional support for children. The chapter also addresses post-disaster recovery and the ongoing psychological care of pediatric patients. It emphasizes the importance of regular simulations, protocol-based training, and interagency collaboration to ensure system-wide readiness. This chapter aims to improve pediatric trauma outcomes in emergency settings by equipping PCPs with knowledge, tools, and structured strategies. It advocates for proactive planning, multidisciplinary coordination, and continuous education as the foundation of effective pediatric disaster response.
