N2 - The authors were invited to provide a summary of the current evidence and an evidence-based approach to practice. Clinical practice guidelines are not available for India and a formal approach to these would require a properly constituted guideline development group rather than an opinion piece from three surgeons. What we have provided is a critical view of current practice and some pointers to a realistic pragmatic approach intended to avoid non-evidence-based practice and excessive harm to no advantage.
